We accept most insurances and are in-network providers for most insurance plans. It is the patient’s responsibility to find out if we are in network with their insurance plan.

Some office visits and treatments for conditions such as hair loss and acne may not be covered by insurance. It is the patient’s responsibility to check with their insurance company for more information as to whether these services will be covered.

Some insurances may require a referral from your primary care physician before they will pay for a visit with a specialist. It is the patient’s responsibility to know this and to acquire the necessary referral forms from their primary care physician prior to the appointment.

Please be prepared to present your insurance card at every visit and notify us of any insurance changes since your last visit. We will typically re-scan insurance cards every six months and after the beginning of a new calendar year, even if there have been no changes to your plan.

Some insurance plans are now charging a higher surgery deductible for select procedures consisting of destructions, excisions and biopsies.  Please be aware that you will be responsible for the out-of-pocket deductible amount if applicable.
